Dr. David Duhon is no longer seeing patients at the Westlake location.
As of August 1, 2015, he joined Guadalupe Sleep Center which is now Texas Sleep Docs.
New or established patients are asked to register by following instructions below.
Please follow these instructions for the following services:
• Medication Refills
• Appointments with Dr. Duhon
• Need a current CPAP or BIPAP supplies prescription
Choose a link below to register yourself as a patient or access your patient portal for Texas Sleep Docs, which Dr. Duhon is now part of.
After you register, our office will confirm you as a patient and email a User Name and Password to log in (this can take up to 1-2 business days)
You are now registered as a patient, however a request has not been made for us to help you.
After you have logged into the Health Portal, you will be asked to log in confirm your date of birth and then reset your password.
The next step is to submit your request. Select “Messages” then select the “General Questions” and enter your request.
** The option of “Refill Request” is available after you have been seen or had a prescription written through this practice. **
Guidelines and expectations:
ALLOW 7-10 days to process your requests, however we will make every effort to complete your request as soon as possible. Please take this into consideration when you are starting to run low on medication(s).
Medication refill requests may require an office consultation if you have not been seen by Dr. Duhon in the past 3 months, unless otherwise specified.
Durable Medical Equipment such as CPAP, BiPAP, and supplies prescriptions are valid for one year. Dr. Duhon has a long-standing policy that patients must see him at least once yearly to remain in his care. If you have not seen Dr. Duhon in the last 11 months, a staff member at Texas Sleep Docs will contact you to set up an appointment. In some cases, he will not authorize prescriptions for medications or CPAP until the patient has made an annual appointment.
